When you look up or call about your coverage, you may want to ask the following questions:

• What are the outpatient pediatric occupational therapy benefits?

• Do I have a limit to the annual number of appointments?

• What is my deductible? Copay or co-insurance for services?

• Do I need prior authorization or a referral.
